I, the parent/guardian of the student named below understand the nature of the events being planned during the School of Popular Music's Summer School.

I am in accord with the purposes of, and procedures, governing the events.

I hereby grant permission for my son/daughter to participate.

We understand that adequate and appropriate supervision will be provided. We further agree to release the School of Popular Music and its employees from liability from unanticipated scenarios that arise outside of the School of Popular Music’s control.
